mental health:
post-traumatic stress disorder (PTSD): a mental health condition that's caused by an extremely stressful or terrifying event — either being part of it or witnessing it. Symptoms may include flashbacks, nightmares, severe anxiety and uncontrollable thoughts about the event

psychosis: a state where an individual experiences a significant loss of contact with reality, often characterized by hallucinations and delusions. These symptoms, along with confused thinking and disordered speech, can affect a person's thoughts, feelings, and behaviors. While psychosis can be a symptom of a mental illness like schizophrenia, it can also result from other causes like trauma, substance abuse, or certain medical conditions

borderline personality disorder (BPD): characterized by a great fear of abandonment, intense and unstable emotions, erratic and impulsive behavior, and paranoia, and it can also lead to psychotic symptoms such as hallucinations and delusions

This is the overall fandom opinion which I relatively agree with. Schizophrenia comes up sometimes but the leaning tends to be BPD which I agree with as the source of her hallucinations given her character.

I tend to err on the side of caution when playing an unstable character unless I'm familiar with who I'm RPing with and/or things have already been discussed, as well as trying to put a cw warning in the tag header. At most without warning she may appear to be talking to herself passively with the intent that she's talking primarily as though instructing to Isha, or dismissing other 'opinions'.
